Neurological Rehabilitation (BNRT)

Short-term community based rehabilitation programme for adults with neurological problems, such as brain injury, stroke and multiple sclerosis.

About our service

Birmingham Neurological Rehabilitation Team (BNRT) provides a short-term community-based rehabilitation programme for adults with a recently acquired neurological condition, such as brain injury, stroke and relapse remitting multiple sclerosis and spinal injury.

 

This excludes progressive conditions and those with an onset of more than 12 months.

 

The team consists of:

  • Speech and language therapists
  • Physiotherapists
  • Occupational therapists
  • Rehabilitation assistants
  • Clinical psychologist

Who is the service for?

Adults (aged 16 years and over):

  • With a neurological condition/disability.
  • Please note: Referrals for people with CVA/ stroke–Community Stroke team (CST) take referrals for patients with a GP in the north, east and west of Birmingham. If unsure please phone to check.
  • Who are registered to a Birmingham GP.
  • Who are ready to start rehabilitation (i.e. medically stable) and have identified goals that can be achieved over a period of 6-8 weeks.
  • Requiring the involvement of 2 or more therapists.
  • Who have a discharge/care package in place (if a hospital discharge).

 

 

What does the service offer?

We aim to offer a co-ordinated interdisciplinary approach:

  • Setting client centred goals to help people maximise their independence. 
  • Reviewing patients’ progress and needs at our weekly multi-disciplinary team meetings and joint working.
  • Rehabilitation in the form of individual and/or group work.
  • Appointments take place in the patient's home, Moseley Hall Hospital or in the community.
  • The provision of support and information for carers, patients and other agencies.
  • Works towards community re-integration (e.g. accessing leisure / educational facilities).

 

 

Who can make a referral?

  • Any health or social care professional, carers, voluntary agencies.
  • Referrals are triaged on day of receipt and the referrer is contacted if more information is required and if the team cannot meet the client’s needs.

What we do at an assessment?

Our client will be interviewed / assessed to establish their needs / goals. This may be carried out in the patient's home, in the clinic at Moseley Hall hospital, by video link or phone, carers/family are encouraged to join with the patients consent.

 

If following this screen assessment, the patient is deemed as non-urgent then they will be placed on the waiting list. (Please contact the team for the up to date waiting time).
