District Nursing

District nurses provide care as part of 38 integrated multidisciplinary teams operating in localities across the city.

Who is it for?

The service is provided for anyone who has been assessed as capable of self-caring and/or receiving care in a domiciliary setting.


Clinical Case Managers

The Clinical Case Management (CCM) service supports patients with long-term conditions who are over the age of 17 and registered with a Birmingham GP.

The purpose of the service is to provide clinical support and education for patients in order to:

1.improve patient / carer long term condition management  

2.support patients to remain at home 

3.prevent unplanned hospital admissions.

CCMs work as a core part of the BCHC adult community nursing and therapy service teams. Operating in this model ensures that patients and referrers have access to a diverse range of clinical support and advice at a locality and neighbourhood level.
